The American Library Association is working with the Hubbard Public Library to improve services for people with accessibility needs. We’d like to hear more from you as a library user about your experiences at the library. Thank you for sharing your thoughts. All questions are optional, and you can skip a question or exit the survey at any time. We will keep the results confidential and anonymize the results for reporting.

There is no compensation for taking this survey. However, we will be contacting some people who fill out the survey to invite them to do a short virtual interview. If you volunteer and are selected for an interview, you will receive a $25 gift card upon completion.

Survey and interview results will allow ALA and our research partner, Knology, to better understand how libraries serve people with accessibility needs.

Providing your contact information is optional and will only be used to select participants for follow-up conversations.
